I am having the same issue. Just installed Intense Debate a few days ago and now a comment I moderated and approved won't show up.

Personally I love ID for comments and post as often as I see a blog using it. Considering it’s a plugin, this piece of software seems to be more at home in Blogger/Wordepress/Typepad than some of the included modules!

I'm having the same problem with IntenseDebate. Most comments are okay, but sometimes after they're approved, they don't show up at all. Anyone know of a fix for this?
